Hyung-tae Kim and Hyun-jin Oh, directors of Yeson Voice Center, presented the effectiveness and treatment results of "pulsed dye laser laryngoplasty" at the 12th Congress of Korean Society of Otorhinolaryngology-Head and Neck Surgery. Document title: The usefulness of 585nm pulsed dye laser with curved laryngoscope in treatments of benign vocal fold diseases
Objective: 585nm pulsed dye laser (PDL), the new treatment method for benign vocal fold diseases, is performed under local anesthesia to preserve the healty tissue, but selects and solidifies only blood vessels at vocal band. The objective of this document is to see the treatment results and the phonetic changes of the PDL surgery.
Method: 18 patients who had received PDL surgery at Yeson Voice Center from September 2005 to January 2006 were enrolled in this study. The patients were followed for an average period of 3.2 months, and their average age was 44. Of the 18 cases of benign vocal fold diseases, 8 cases were vocal hemorrhagic polyps, 3 were laryngeal granuloma, 2 were papillomas, and 5 cases included laryngeal leucoplakia and blood ecstasia. 2 months after the surgery, improvements of lesion and the voice were checked with digital laryngoscopy and voice analysis, and the subjective satisfaction of voice was measured. The significance test was done through the Wilcoxon Signed Ranks Test.
Results: In the PDL surgery using curved laryngoscope, no complications related to anesthesia or surgery were observed. 83% of the patients (15 cases) had 70% of lesion was removed, and 17% (3 cases) showed 50-70% removal of lesion. In voice analysis, sPPQ and sAPQ showed improvement in significance test. Subjective satisfaction of patients increased in 78% of patients (14 cases), stayed the same in 22% (4 cases), and relapse was seen in none of the cases. Complications of postoperative petechia, sputum, sense of foreign substance, cough, and congestion occurred in every case, but no bleeding occurred.
Conclusion: PDL using curved laryngoscope can be performed under local anesthesia with no complications. PDL is a bloodless operation which provides great treatment results in serious vocal fold diseases.
